If your renewal is completed correctly, you will receive a confirmation e-mail.
Before starting your online renewal, you will need to have a copy of the following electronic documents so they are ready to upload: CPR/FA, Practical Hours form, and the Continuing Education Credits form. You can submit a scan of these documents, or take a picture of the documents with your smartphone.
In the past year (January - December), please provide an estimated number of individuals who attended your fitness classes. If a person is in multiple classes, count them only once.
